On November 20, 2023, delivery driver Chris Keegan was operating the defective forklift in the dark to reverse it onto a trailer at a customer's site in Sheffield when it fell, flinging him from his seat and trapping him between the chassis and a neighbouring trailer, resulting in his death at the scene.
Pleaded guilty to breaching Section 2(1) of the Health and Safety at Work etc. Act 1974. Ordered to pay £8,146.80 in costs and a £2,000 victim surcharge.

The incident was deemed easily avoidable with proper monitoring and supervision.
Pleaded guilty to breaching Regulation 5(1) of the Management of Health and Safety at Work Regulations 1999. Ordered to pay full costs of £5,768.06. Company accepted full responsibility and conducted a comprehensive health and safety review post-incident.
