On November 20, 2023, delivery driver Chris Keegan was operating the defective forklift in the dark to reverse it onto a trailer at a customer's site in Sheffield when it fell, flinging him from his seat and trapping him between the chassis and a neighbouring trailer, resulting in his death at the scene.
