Sawmills Limited's premises in Ellesmere, Shropshire, in May 2021. He climbed onto a stationary conveyor bed and used a metal pole to move a stuck log in the quad saw machine. The conveyor started unexpectedly, dragging his legs across moving chains and trapping them against a stop plate. An HSE investigation found the company failed to prevent access to dangerous parts, stop machine movement in danger zones, and provide safe systems of work, information, instruction, training, and supervision.
